3. Are you running the ones where it happens in full screen mode?
Windows File Explorer yesWith Explorer (as in Windows File Explorer), there is no full screen mode, so that is odd, for sure.
Since you also mention the updated stack package, are you running a Developer / Canary Insider's build?

I'm not currently running a Canary build, not even in a VM, as I cleanly installed just 2 weeks ago, but it would be interesting to see if you can grab and move the window around, and / or use the Win + Arrow keys to move it into the pre-defined split views, and see if the buttons come back.
Also, have you use ViveTool to unlock anything on your Canary install? And,. related, but do you have any programs that alter things like the taskbar / start menu installed?

AFAIK, it's a minor bug in Explorer. It happens sometimes (not always). Sometimes you address Explorer from a 3rd party app like Edge or Picasa (open file in Explorer option), and it happens. There is a "tab close" button (x), not the main View attachment 64279 buttons are there.
Another bug that happens is whenever you open Explorer from the Edge ("Open downloads folder" menu), it automatically enables "Show Navigation Pane"! I have "Show Navigation Pane" disabled.
