I have a strong feeling both of these issues were do the the explorer extension that comes with Notepad++.
It is not something that shows up in the nirsoft tools, so I never tried to disable it. It is also one of the very first things I install on my PCs.
Even after uninstalling it, it still seems to be in the explorer menu, even though it no longer links to anywhere. There is an option to not install the explorer extension though.
I had to actually unregister it's Nppshell.dll with regsvr32 to remove it. Everything has been working fine since then, at least for now...
