Some updates need to be applied outside Windows... some don't.
You'd need an MS programmer to explicitly answer your question.

I guess it's like registry edits. Some require that you do nothing, some want you to sign out, some want a reboot.

Every .Net update in the past has required a restart. I thought at first maybe MS had changed the WU system to not request a restart while the Cumulative update was installing but that wasn't the case - the .Net update had installed (shown by Reliability Monitor).

I've had some that didn't require a reboot and other that did, but were happy to wait till the WU was finished installing.
