OS
win11 and 10
for many programs I am in use of ctrl + ~ shortcut.

to name a few:
- notepad++
- QTtab bar
- firefox page translation
etc

after using windows for awhile these key stopped working, though other hotkey still function properly. I closed all the tasks in task manager, down to only the ones I start with when computer boots up but still doesn't fix this issue. a restart of computer does fix this, but restart explorer.exe does not fix this.

has anyone came across this issue? any idea?

Try these suggestions.

1. First I would suggest you make sure you have no corrupt system files that is causing this problem. Open a command prompt as administrator and type sfc /scannow
Pay attention to the scan results to see if SFC found anything it could not repair. If it found corruption it could not fix followup with this command (note the spaces)
dism /online /cleanup-image /restorehealth
Restart computer

(Note-you can follow up with another sfc /scannow command to make sure that dism repaired the files)

a restart of computer does fix this, but restart explorer.exe does not fix this.

2. This would lead me to think you have some kind of software conflict.
Perform a clean boot to eliminate software conflict causing the issue. If it doesn't happen in a clean boot, you have some application conflicting with Windows. Brink's procedure will help you weed out which one. Perform a Clean Boot in Windows 11 to Troubleshoot Software Conflicts Tutorial

To shorten the procedure, you might first eliminate any of your startup applications to see if any of them is the culprit.

after 1.5 years finally found the issue lol

issue is shift + alt being pressed, and its shifted to another language? (though when typing it is still english) but for some reason causing ctrl + ` to stop working.

Thank you for reporting back.. Back in the day, the shift+alt KB shortcut changed languages if an additional language was defined, but I was under the assumption that key combo no longer worked for that function. Guess I was wrong.
