Unknown page interrupts printout

What is ths page? About half the time when I request PRINT, the first thing to print is this page, then the page(s) I requested. How do I get rid of it? Don’t know if it’s a PC problem, a printer problem or a software problem.

Does it happen from any program or just a specific program?
My thoughts go to a program specific setting or a printer specific setting more likely the former if it doesn't happen all the time. Can't say I've ever seen something like this print.

I did an experiment. I printed out an email posted to me. No mystery document. I sent meyself an email and printed it, No mystery document. I created a document (Exterminator) and did not send- just printed it. The mystery document printed out first.

Sometimes when seeing weird things with printers it can be helpful to pull all cables to the printer to clear its cache/memory and reboot the computer, try again. Might also check the program and printer for a setting about a diagnostic or summary page being included.
