I have seen this occasionally, including twice in the past ten minutes or so, when the Open Shell icon appears in the middle of the task bar (I have program icons moved to the left from the default center location), and the built-in Windows start button appears at the far left of the task bar. Clicking the Open Shell icon restores it to its proper place and the Windows start button is banished.
The first time this happened was rather concerning, as I feared that something serious had taken place; now,, at least, I know that it only takes a mouse click to restore the icon situation, but it is rather puzzling.
