PC is freezing when installing 4090

I am gonna give the single RAM card a shot. In the included pictures is what I mean by the special 3 to 1 dongle, There is no other input on this 4090. Also, the different ways I was using the y splitters simply because I only have 2 straight-through VGAs and was forced to use at least one y splitter. I have used them with only one plug on the double side being used and tried it with both on the double side of the Y being used on the PSU (effectively 5 VGA slots to three cables to the dongle down to one cable) hope that makes some kind of sense lol. Either way same result. I removed the other cables for the visual clarity of the pictures.
It looks like you have the cables connected correctly (except picture #1 - that is not correct).
3 cables into the power supply's 3 VGA ports, 3 cables into the adapter.

While it makes no difference electrically, the PCIe cables are usually run the other way - the single 8-pin connector into the power supply, and the 6+2 connector into the video card. The Y branch at the end could be used to populate two power ports from one cable. I'm not sure if that is recommended in your case. But the way you have it will work and looks better in the case. But you don't plug both of those into the power supply like picture #1.

I'm wondering if the 12 pin RTX adapter came from MSI with the card, or is it from EVGA with the power supply?

An update.... I have called both EVGA and MSI about the wiring and they assured me that everything was done correctly from how I described it to them. The new 4090 came in this morning and I tried it out. It does the exact same thing as the last. I tried with only one ram card and with 2 cards same result. I retried the steps that GHOT laid out for the DDU and driver reinstallation. did not work. I reluctantly reimaged and reset everything. still does the same thing and freezes everything. I am contemplating switching out the motherboard for an ASUS Z690 extreme to see if an older gen chipset will work. Along with swapping out the RAM cards for something that is definitely verified for DIMM 4 compatibility. At this point my spirit is broken lol, I don't know what to do troubleshooting-wise besides throwing money at this damn thing in the form of new parts.

If anyone has any last-ditch efforts for me to try I would greatly appreciate it.

It should be enough. The thermals for the CPU are very constant and don't spike at all. perhaps once I start gaming or putting it under load I'll find out lol. But for just the setup and troubleshooting I've done its been fine.

In your OP you mentioned that you "reimaged" multiple times both for Win11 and Win10. When you use the term "reimaged" what exactly are you saying here? Reimaged is generally a term used when one is making a backup image of the drive. Or do you mean you reinstalled either WIn11 or Win10?

Honestly I find it hard to believe you've had a bad motherboard and 2 bad GPUs.

As I reread the whole thread I'm a little cautious the way you've connected the GPU power from the power supply. The PCIe cables are meant to be installed with the single connector AT THE power supply while the split ends are(or were) meant to be connected at the GPU power connectors. Over time as the power requirements grew for more power hungry GPUs to use of the split ends became an issue. That's why on newer power hungry GPUs it is recommended to only use one of the ends and to use a separate PCIe cable for each power connector on the GPU.

Also if you're using the single 120 AIO make sure the radiator is standing up and not laying flat to allow fan cooling to easily flow "through" the radiator. It appears in the photo it's laying down flat preventing air flow.

I mean completely removing and reinstalling either WIN 10 or 11. In the pictures, the fan does look flat but is indeed propped up a little over an inch. Also, for the wiring, I have set them up in both configurations. the single end from PSU and only using the main branch on the Y end, effectively 3 PSU ports to the 3 ports on the GPU 12pin dongle that is shown in the pics (using 2 straight throughs VGA cables and 1 Y cable). And I have also set up the Y side from the PSU to the 3 ports on the dongle. Effectively 5 PSU ports to the 3 GPU dongle ports, Neither makes any difference. I also don't like the idea of having replaced the Motherboard once already and that my new GPU is doing the same. I don't think it's either anymore. I called a few repair shops in my area and have been able to pick their brains a bit about my situation. 2 different shops told me essentially the same thing, that they have seen the blue screen error I get " WHEA_UNCORRECTABLE_ERROR" a lot lately and in there experience has been caused by one of two things. Either the GPU OR the CPU. They have had cases where they have replaced either the GPU or the CPU or both and have corrected the problem. I put an order in for a new CPU, a new PSU with the 12v power port specifically for PCIE 5 built-in, and new RAM cards that are actually verified on the compatibility support page of the motherboard.

I will add a few more pics for clarification

I wasn't sure, so I checked a cable (single PCI-E VGA) with a digital voltmeter.

The pinouts are not the same at both ends. It's too bad that eVGA labels both ends "VGA", at least for the cable I grabbed. It's also too bad that it's possible to plug the graphics card end into the PSU and vice-versa.

I hope that you haven't fried anything by plugging the wrong end of the cable into the PSU.

Oh jesus. Well.... me too.

There are no warnings, but the manual says:

Q. Does it matter which end of the cable I plug into the power supply?
A: Yes, the connections going to the power supply will not have “break downs” like the EPS (CPU) has 4+4 on one
side and full 8-Pin on the other. Check each cable as it is labeled appropriately to match the power supply side,
and connect only the same ‘‘matching” cable directly to the power supply as referenced on each connector.

Ahh ok, I have plugged them in correctly now. Unfortunately still doing the same thing. I am going to run Windows memory diagnostic tool to see if there are any hardware issues.

I feel like the guy in the video is taunting me lol. " check out how good this GPU is!"
Also, I installed it similarly to him, but didn't mess with the RGB wires. Wonder if he ran into any problems.....

This may not help, but check my system 1. (Gigabyte RTX 4090 OC.)

The main difference is that the 4090 is connected to the PSU using an aftermarket (Cablemod) cable.
