Solved Windows 10 to Windows 11 upgrade. BSOD with FLTMGR.sys system tread exception not handled.

So I’m trying to preform an in place upgrade from Win 10 to Win 11. But no matter what method I try I get the BSOD mentioned in the title and it rolls back to Win 10. Anyone else figured out how to get passed this?

Let's do the basic
Open a CMD window as administrator and type:
chkdsk c: /f
It will say your disk is in use and ask if you want to schedule to next start = yes
Restart

Back to Windows, open a CMD window as administrator and type:
sfc /scannow

If it finds any corrupted files, fixing or not, reboot and run again

Then run
Dism /Online /Cleanup-Image /ScanHealth
and
Dism /Online /Cleanup-Image /RestoreHealth

A successful win 10 to win 11 upgrade also depends on the type of the drive initialization (MBR or GPT)
Please post a whole window Disk Manager image of ALL your drives. Don't forget to expand the columns so we can read them. How to Post a Screenshot of Disk Management
If you have a MiniTool or AOMEI Partition use it instead or Windows disk manager.

I have some strong XP computers. and tested OS install of W7 (x86, x64), W10 (x86, x64), and W11. running W7 (x64) and W11, often got same BSOD. W7 (x86), W10 (x86, x64) were ok. I was thinking W10 x64 should be crashing too.

Anyway, I let them staying with W10 (x86 or x64. no matter). more stable and compatible comparing to W11 or W7.

Your specs say that the processor is a Intel(R) Core(TM) Ultra 5 235, which is the latest generation released Q1 2025. Yet your Windows level is Win 10 22H2 Build 19045.6093. Is this a self-build or a Dell supplied machine? If the latter, I would have thought it came with W11 pre-installed.

Also your disk information shows the C drive to be 100% full.

Perhaps a clean-install of Windows 11 might be a better option?

Yes you are right, this Dell PC did come preloaded with Windows 11. I Syspreped a Windows 10 PC and moved the image over to this PC so I could capture all the data and applications. Now I'm trying to upgrade it to Windows 11 and I'm running into issues. The C drive is not full. Must have been bitlocker being enabled that was not showing the correct usage. I know I could do a clean install but I was hoping someone on this forum has possibly run into this problem and could offer assistance.

To find what is causing the upgrade failure, you have to look at the logs. Different logs are generated at each phase of the upgrade.
Posssibly, something will jump out at the time of the rollback. They are in Microsoft speak so I can't say if everything can be interpreted.
When I was upgrading 23h2 to 24h2, Mine kept rolling back so I found my particular error in the last 2 logs. It indicated there was some issue with my user account but I could not decipher what the issue was so I ended up clean installing. Hopefully you will have better luck. These instructions came from MS.

  • WIINDOWS UPGRADE FAILURES
If no error message occurs, the following log files are created when an upgrade fails during installation before the computer restarts for the second time. You have to look at these logs to get an idea of what's causing the failure.

The following log files are created when an upgrade fails, and the installation rollback is initiated:

If logfiles do not direct you to what caused the error, I advise anyone to not waste his time. Either stay where you are or bite the bullet. back up your data, and do a clean install. Upgrade failures will have you chasing your tail for days.

Post clean install please consider the following steps:



1) Add WMIC



2) Run Tuneup plus




3) Add Regback



4) Create a new restore point
