Windows keeps loosing exe icons

I don't know what is going on but after I clear the icon cache,it doesn't take long until I loose the icon again on some programs
Instead of showing the exe actual icon in the taskbar,it just shows the default exe icon on(programs that don't have icon build in)

I did the usual scanning for errors but there was nothing wrong

So, this is specifically about icons in the Taskbar, right?

What about icons in other locations for the same programs? Do program icons in Start Menu also change? What about icons in File Explorer, if you go to the actual physical location of the executable?

As for the Taskbar: are you talking about pinned icons? Or are you talking about icons of programs that are simply running (but not necessarily pinned to Taskbar)? Or both?

Yes the icons are wrong only on the taskbar,everwhere else(star menu,creating a shortcut or opening the exe location)the icon is correct
And no,the programs are not pinned

Try disconnecting OneDrive if you use it.
Have you tried unpinning, right-clicking the app, properties, change icon?
